Insurance
If you have orthodontic coverage on your dental insurance we will be happy to file for you once treatment has started. We ask that you complete the personal information portion of the claim form and we will coplete the clinical information. We will mail the claim in once treatment has started.
Our office does not accept insurance assignment , therefore any orthodontic benefit will be mailed directly to the insured.
